if your just looking for calories... I would go with a combo of full fat milk, whey protein, oatmeal, and a banana... i dont know how that breaks down exactly, but assuming that you are using 12oz of milk, 50 grams of protein, 1/2 cup of oatmeal, and a large banana thats gotta be running close to 600 cals
if you need more then that you could throw in some yogurt and juice

2 cups full fat milk
.5 cup oats
2 scoops vanilla protien
.5 can pinapple
.5 banana
thats over 1000 kcal and tastes great.
peanut butter is good in chocolate flavored shakes. olive oil or heavy whipping cream are also excellant sources of calories. be creative, i know guys who use pie filling in their shakes. greg valentino used to put slices of cake into his protien shakes. a really tasty option is to blend promax bars into your shakes but gets expensive.

a couple years back a guy gave me a recipe for a shake, called swole. if i remember correctly:
2 cans of sweet pottaoe(soft)
1 bannana
2 tbsp of peanut butter
vanilla (optional)
and 20+floz of milk
and Blend
you can had a little suger for taste (brown).
can also add some protien powder but the shake will most likley turn out thick.
